For many Latino families, pregnancy is a time filled with joy and anticipation. However, Latina women in the United States face several unique challenges during pregnancy and childbirth. These challenges, often rooted in systemic issues such as barriers due to language, cultural differences, and financial instability, contribute to disparities in maternal health outcomes for this population, according to the Policy Center for Maternal Mental Health.

Maternal mortality rates in the United States are troubling across all demographics, but they are particularly high for minority groups. In 2023, Latina women experienced a maternal mortality rate of 32.9 deaths per 100,000 live births, according to the Centers for Disease Control and Prevention (CDC). Although this rate is lower than of Black women, it highlights the ongoing disparities within maternal healthcare, and the need for tailored, culturally competent care for pregnant Latinas

Research shows that Latina women often face obstacles that increase their risk of maternal complications. These barriers include delayed prenatal care, underutilization of maternal health services, and less access to specialized care during pregnancy. Furthermore, Latinas are more likely to experience higher rates of preterm births and low birth weight, which are key indicators of complications during childbirth. These barriers are often cultural, economic, and language challenges that prevent Latinas from receiving timely, appropriate care (Centers for Disease Control and Prevention, 2023).

Doulas transform the birth experience for Latinas

A promising solution to this problem is the increased involvement of doula-trained,non-clinical professionals who provide emotional, physical, and informational support to mothers during pregnancy, labor, and after childbirth. Doula care has been proven to reduce maternal health risks, improve birth outcomes, and provide a sense of empowerment to mothers, particularly those from marginalized communities.

For Latinas, the benefits of having a doula extend beyond support during labor. Doulas are essential in providing culturally relevant care that honors the traditions and values of Latina families. Bilingual doulas, who speak Spanish and understand the unique cultural context of Latina mothers, are crucial in ensuring that these mothers feel heard, understood, and fully supported throughout their pregnancy journey.  

Doulas offer physical support during labor and help ensure that Latina mothers receive proper prenatal and postpartum care. They also advocate for mothers in medical settings, assist with communication and help navigate complex systems. According to U.S. Department of Health and Human Services, research shows that having a doula increases a mother’s sense of control over her care, leading to fewer complications and better birth outcomes.

“Improving maternal health outcomes for Latino families requires a commitment to understanding and addressing their unique needs during the pregnancy journey”, said Valerie Martinez, chief health equity officer for Blue Shield Promise.  “By expanding access to doulas and ensuring culturally relevant care, we support mothers to make informed decisions, advocate for themselves, and feel supported throughout their pregnancy journey.”

Blue Shield Promise recognizes the importance of doula support in improving maternal health outcomes. Through its Doula Service Initiative, the health plan is working to ensure Latina mothers have access to this essential service. This program provides equitable maternal health care by:

  • Training bilingual doulas to serve Spanish-speaking mothers

  • Providing Medi-Cal-covered doula services for all eligible mothersfamilies

  • Raising awareness of the benefits of doula support among Latina communities

  • Collaborating with local organizations to improve healthcare accessibility in underserved areas

Postpartum Care: A Vital Component of Doula Support

The role of doulas doesn’t end with delivery. In fact, postpartum care is just as critical. Latina birthing persons, like many new parents, face emotional and physical challenges during the postpartum period. Doulas provide crucial support by offering advice on breastfeeding, postpartum depression, and other aspects of recovery. This comprehensive care helps mothers feel supported in the early days of motherhood, easing their transition and ensuring they have the resources to thrive.

The path to health equity for Latina mothers

With initiatives like Blue Shield Promise’s Doula Services Initiative, more Latino families will have access to the vital care and support they need throughout their pregnancies.

“As doulas work to bridge gaps in maternal health care and provide culturally competent care, Latinas can expect healthier, safer, and more empowering pregnancies and births. With increased access to doula care, we can move closer to achieving health equity for all mothers,” added Martinez.
